Why Staying Connected to Your Community Matters
Remaining connected to the wider community is not a luxury — it is a fundamental component of physical and mental wellbeing. The ability to attend a medical appointment independently, to browse a market, to visit a place of worship, or simply to share a coffee with a friend in a local café, has an enormous impact on how individuals feel about themselves and the world around them.
When reduced mobility, cognitive change, or the loss of driving ability cuts off access to these experiences, the effect can be profound — increasing isolation, reducing confidence, and accelerating physical and mental decline.
Our community access service exists to rebuild and maintain that connection — safely, reliably, and entirely on the individual's terms.

Who Benefits From Community Access Support?
Individuals Who Have Lost Their Driving Licence
For many older adults, giving up driving represents a significant loss of independence. Community access support restores that freedom — enabling individuals to go where they need and want to go, on their own schedule.
Those with Reduced Mobility or Physical Conditions
Arthritis, Parkinson's, stroke recovery, and other conditions can make independent travel difficult or unsafe. Our carers provide the physical support and confidence individuals need to get out safely.
Individuals Living with Dementia
Community outings — familiar places, familiar routes, meaningful activities — are enormously beneficial for those with dementia. Our carers provide safe, consistent accompaniment that maintains community connection without risk.
Those with Anxiety or Low Confidence Outdoors
Some individuals become fearful of leaving the home following a fall, a health event, or a period of illness. Community access support rebuilds confidence gradually — at the individual's pace.
Individuals Without Nearby Family Support
For those whose family members live at a distance or are unable to provide regular accompaniment, community access support fills a vital gap — ensuring outings still happen, safely and reliably.

How It Works — Simple, Reliable, Person-Centred
Tell Us What Matters to You
During our free home assessment, we discuss the individual's community interests, regular commitments, and the outings that mean most to them. We plan around their life — not around a generic schedule.
We Match the Right Carer
We match the individual with a compatible carer who understands their pace, their preferences, and how they like to be supported outdoors. Continuity of carer is maintained wherever possible.
Life Outside Begins Again
Outings are arranged at the individual's preferred times. The carer collects, accompanies, supports, and returns them home safely. Families are updated and any concerns reported promptly.
What Families and Clients Tell Us
“Since Mum stopped driving two years ago, she barely left the house. Her Cornerstone carer takes her shopping every Thursday and to her church group on Sundays. The difference in her mood and energy is extraordinary — she is herself again.”
“Dad has Parkinson's and had become too nervous to go out after a fall. His community access carer builds his confidence visit by visit. He went to the allotment last week for the first time in a year. That means everything.”
