Why Medication Management Cannot Be Left to Chance
237,000

50%
Of adults aged 65+ in the UK take five or more prescribed medications daily
(NICE, 2023)
1 in 3
Medication-related hospital admissions in older adults are considered preventable
(BMJ, 2023)
These figures represent real people — individuals who missed a dose, took the wrong medication, or received no support in managing a complex regime. For older adults managing multiple conditions, medication errors are not a minor inconvenience. They can be life-altering.
At Cornerstone Care Ltd, medication management is treated with the clinical seriousness it deserves — because the stakes demand nothing less.

Our Medication Management Services
Clinical Note: All medication support at Cornerstone Care Ltd is carried out within our documented governance framework. Our Registered Manager — a qualified Registered Nurse — oversees all medication policies, staff training, and competency assessments. No carer administers medication without completing our mandatory medication training and competency sign-off.

How We Keep Medication Safe — Every Single Visit
Every medication administration at Cornerstone follows our four-step safety protocol — without exception.
Check
The carer checks the individual's MAR chart before every administration — confirming the correct medication, dose, time, and route. Nothing is assumed. Everything is verified.
Administer
Medication is administered precisely as prescribed. The carer observes the service user taking the medication where self-administration is supported, or administers directly where required and authorised.
Record
Every administration is recorded immediately on the MAR chart — time, dose, carer initials, and any observations noted. No gaps, no delays, no assumptions.
Report
Any concerns — missed doses, side effects, changes in behaviour, or stock discrepancies — are reported immediately to the Registered Manager and, where appropriate, to the service user's GP.

Clinical Oversight at Every Level
Medication management at Cornerstone is not simply a care task — it is a clinical responsibility. Our governance framework ensures it is treated as such.
Registered Nurse Oversight
Our Registered Manager is a qualified Registered Nurse with 15+ years of NHS clinical experience. All medication policies, training, and competency assessments are overseen and approved by her directly.
Mandatory Training
Every Cornerstone carer completes our medication training programme before administering any medication independently. Competency is assessed and signed off — not assumed.
MAR Chart Documentation
Medication Administration Records are maintained for every service user. These are reviewed regularly by our Registered Manager and are available to family members and clinicians upon request.
Regular Reviews and Audits
We conduct regular medication audits across our service, identifying any discrepancies or concerns early. Changes to prescriptions are managed carefully with GP and pharmacy communication.

Who Benefits From Professional Medication Support?
Older Adults on Multiple Medications
Individuals managing five or more daily medications — a common scenario for those with multiple long-term conditions — who benefit from professional oversight to prevent errors and missed doses.
Those with Memory Conditions or Dementia
Individuals with dementia or memory conditions who may forget to take medication, repeat doses, or confuse different prescriptions without professional supervision and prompting.
People Recently Discharged from Hospital
Individuals returning home after a hospital stay, where medication regimes have often changed, doses have been adjusted, and close monitoring is essential during the recovery period.
Those with Complex or High-Risk Medication
Individuals on medication with a narrow therapeutic range — such as anticoagulants, insulin, or certain cardiac medications — where accurate dosing and close observation are critical to safety.
Family Carers Concerned About Safety
Family members who have noticed missed doses, confusion around medication, or stockpiling — and who need professional, reliable support to take over this responsibility safely.
